Your first visit will include an examination by the dentist, followed by an assessment and scaling with the hygienist.  This can be booked on the same day or in two separate visits.  At this visit, we will diagnose any dental and/or periodontal (gum) areas of concern, as well as provide a recommended treatment plan.  We will also address any dental concerns or questions you may have.

If you have been to another dental office in the last 3 years, you will be asked to sign a form requesting a copy of your records be sent to our practice.  Otherwise, your examination will include written, computer and digital x-ray records.

A new patient Medical History Questionnaire is available for download (see PATIENT Forms page) and can be completed prior to arriving for your appointment.

Our office accepts Company sponsored dental insurance plans and takes assignment of benefits directly from the insurance company.  However, it is always the responsibility of the patient to pay any account balances not covered by insurance.

Note:  All new patient emergencies must pay in full at the time of their visit.  There is no assignment of benefits for emergency only patients.

Our front desk staff will be happy to assist you with any questions you may have about your dental coverage. It is important to inform our staff of any changes to your dental coverages.  In many instances, the insurance company will not release patient information directly to the dental office.

A pre-determination of benefits may be requested from your insurance company to determine treatment coverages, and to determine if yearly maximums will be exceeded.  A pre-determination is also beneficial for larger treatments, in order to budget for any uncovered expenses.
